Read Me - Important Information

The state of IN requires us to prove the validity of businesses we work with. As such, we require a copy of relevant paperwork, which may include but may not be limited to your Business License and/or Resale Tax Certificate.

When applicable, we require a copy of your FFL.

Digital copies of the application may be released to info@damageindustriesllc.com and physical copies may be mailed to the address below.

Step 1: Install Adobe Acrobat Reader - It's FREE!  Click HERE to download.

Step 2: Open the file "Damage_Dealer_App" and select the TOOLS tab.

Step 3: Select "Fill & Sign".

Step 4: Click anywhere you need to place information.

Step 5: Add your signature.
